Humana Military (Tricare) requires patients to obtain a referral from their PCP to any specialist they wish to see.
The referral request can be submitted from the Humana Military site or by faxing a completed Patient
Referral Authorization Form. After Tricare approves the referral then the patient can be seen by the specialist. The specialty provider can then order genetic testing. Once we receive the order from the specialist we can proceed with our request to Tricare for preauthorization of testing.
If Tricare determines that genetic testing is not medically necessary, we will contact the ordering specialist and ask that they speak with the patient on whether they want to proceed with testing and pay out of pocket. We will ask that a Non-Covered
Services Waiver be completed and signed by the patient and then submitted to our laboratory. If the patient does not want to proceed with testing the orders are then cancelled.
